Abstract

Recent studies based on histological and MRI examination have improved the understanding of the pathogenesis of osteonecrosis of the femoral head (ONFH) (Hauzeur, 1989). The early stages have been more systematically explored. Until the presence of typical radiological lesions with the crescent sign and collapse, radiology could be doubtful or normal. In such cases bone scintigraphy is positive in 51%, MRI and biopsy in 100 %. Excessive intraosseous pressure is found in 80 % of the early stages. This data led to the division of the evolution of ONFH into three phases. First, the necrotizing process produces areas of bone weakening or bone thickening. Such remodelling induces modification in the mechanical properties of the bone. The second phase results from the bad outcome of the first one when a fracture of the subchondral bone arises, usually in the weightbearing portion of the head. The third and last phase is osteoarthritis produced by acquired dysplasia due to the fracture.
