Adherence to the Medical Regimen in Transplantation

  • Chapter
Biopsychosocial Perspectives on Transplantation

Abstract

Adherence to the medical regimen both before and after receiving an organ transplant is of obvious importance for maximizing one’s health and well-being. The increasingly sophisticated medical therapies and technologies available to prolong life - including organ replacement and organ supplementation by mechanical means - still require an active investment on the patient’s part in order to obtain the best outcomes. Transplant candidates and recipients are asked to engage in a large variety of behaviors, including medication-taking, monitoring vital signs, attending clinic appointments, following exercise and diet plans, and abstaining from or limiting substance use. Nonadherence - or noncompliance - i.e., the degree to which behavior fails to coincide with medical recommendations in each of these areas (Cramer & Spilker, 1991; Haynes et al., 1979), has the potential to place these patients’ health at risk. (Although distinctions are sometimes made between the terms, nonadherence and noncompliance, we will use both interchangeably to connote the behavioral definition above.)
