Abstract

Many classifications have been proposed for anal fistulas throughout the years. These include, but are not limited to: high vs. low, simple vs. complex, and primary vs. recurrent. In 1976 Park, Gordon, and Hardcastle proposed a detailed classification of the fistulas based primarily on the involvement of the internal and external sphincter in the fistula. This classification is not only useful in guiding the surgeon to make an educated decision at the time of surgery, but also allows the surgeon to inform the patient on the complexity of the disease process, chance of success, and possibility of postsurgical sequela, especially incontinence. Also this classification has been instrumental in the development of various surgical techniques and use of biologic and synthetic products for preservation of continence.
