Zusammenfassung
M. ist das erste Kind gesunder Eltern. Nach unauffälliger Schwangerschaft erfolgte die Geburt per Sectio bei Beckenendlage in der 39. Schwangerschaftswoche. Im Alter von 6 Monaten fi el den Eltern erstmals beim Kind eine leichte Behaarung des Penisschafts auf, mit 9 Monaten entwickelte sich eine Akne im Gesichtsbereich. Im Alter von 12 Monaten erfolgte beim Kinderarzt aufgrund der Akne und des auffälligen Genitale mit Penisvergrößerung und Schambehaarung eine Sonographie des Abdomens, wobei ein Tumor im Bereich der rechten Nebenniere entdeckt wurde.
Nach Einweisung in eine Universitätskinderklinik zur weiterführenden Diagnostik und Therapie stellte man einen adoleszent vergrößerten Penis mit dunklen Flaumhaaren an der Peniswurzel fest, die Hoden waren beidseits deszendiert mit einem Volumen von ca. 2 ml, das Skrotum war dunkel pigmentiert. Bei der Bildgebung fand sich im Bereich der rechten Nebenniere eine etwa 9×5×7,5 cm große, relativ gut abgegrenzte Raumforderung mit einem Volumen von etwa 200 ml. Die Hormondiagnostik zeigte stark bis massiv erhöhte Serumspiegel von Testosteron, Dehydroepiandrosteron, Dehydrotestosteron und 17-Hydroxyprogesteron. Es bestand kein Anhalt für eine Metastasierung. Da eine primäre vollständige Tumorentfernung nicht möglich war, erfolgte vorerst eine Probeentnahme. Die pathohistologische Aufarbeitung ergab die Diagnose eines Nebennierenrindenkarzinoms. Nach erfolgreicher neoadjuvanter Therapie nach dem GPOH-MET-97-Protokoll (Verkleinerung des Tumors auf 20% des ursprü nglichen Volumens mittels Polychemothe rapie und anschließend kompletter chirurgischer Entfernung des total nekrotischen Tumors) ist der Knabe 2 Jahre nach Therapieabschluss in anhaltender Remission.
