Key Points
1. Gastro-gastric fistula (GGF) is a rare complication in laparoscopic divided Roux-en-Y gastric bypass (RYGB).
2. GGF can lead to weight regain, poor weight loss, or marginal ulcers.
3. We report a practical approach of combined laparoscopic gastric pouch resizing with fistula resection and distalization of the bypass in a patient with a GGF and weight regain after RYGB.
