Hintergrund
Durch die Zunahme psychosomatischer Komorbiditäten rückt die Behandlung psychischer Störungen als prognose- und therapiebelastende Faktoren zunehmend in den Fokus des Interesses von Schmerzambulanzen.
Ziel
Mit der vorliegenden Untersuchung sollen die Vorteile einer liaisonpsychiatrischen Betreuung von Patienten einer universitären Schmerzambulanz aufgezeigt und diskutiert werden.
Methode
Einbezogen in die retrospektive Untersuchung wurden alle Patienten, die sich von Januar bis Juni 2014 in einer anästhesiologisch geführten Schmerzambulanz vorstellten. Die psychiatrische Anamnese wurde von einem in der Schmerzambulanz tätigen Liaisonpsychiater erhoben und dokumentiert.
Ergebnisse
Im untersuchten Zeitraum befanden sich 485 Patienten in ambulanter Behandlung. Insgesamt 351 Patienten (72,4 %) konnte eine psychische Diagnose zugeordnet werden. Die Verteilung der diagnostizierten psychischen Störungen war mit einem konsiliarisch tätigen Dienst vergleichbar. Der Anteil der Patienten mit Anpassungs- und affektiven Störungen dominierte. Die Therapie erfolgte vorzugsweise mit Antidepressiva der neuen Generation.
Schlussfolgerung
Die ständige Präsenz eines Liaisonpsychiaters ermöglicht eine zeitgerechte fachspezifische Mitbetreuung der Patienten im Sinne eines multimodalen Therapieansatzes.
Abstract
Background
Owing to a rise of psychosomatic comorbidities, the treatment of psychological disorders, which may negatively impact prognosis and therapy, is increasingly becoming a focus of attention for pain outpatient clinics.
Aim
This study investigates and discusses the advantages of liaison psychiatric care in a university pain clinic.
Methods
In this retrospective study, we investigated all patients who presented to an anaesthesiologically led pain clinic between January and June 2014. The psychiatric history was taken by the liaison psychiatrist of the pain clinic.
Results
In the period investigated, 485 patients were treated as outpatients. A psychiatric diagnosis was present 351 patients (72.4%). The distribution of the diagnoses was comparable with that of a consultation service. Adaptation and affective disorders dominated. The patients were preferentially treated with new generation antidepressants.
Conclusion
The constant presence of a liaison psychiatrist allows for timely, specialised care of pain patients in terms of a multimodal therapeutic approach.
