Abstract

Incidences of emerging viral pathogens account for global public health concern with a large proportion of morbidities and mortalities worldwide. The novel viral pathogen may be potent of contagious epidemic or even serious pandemic, clinical prognosis of which often relies on early detection of causative agent. Emergence of pathogenic viruses in the modern world dates back to influenza pandemic of 1918, with millions of casualties worldwide, followed by SARS CoV in 2003, Swine flu in 2009 MERS CoV in 2013, Ebola in 2015 to the most recent SARS CoV-2 pandemic in 2019. Despite significant advances in understanding etiology of viral pathogens, there is scant information on the perpetual nature of their emergence, pathogenesis and transmission. Hence, quick and accurate detection of the viral agent is the first step to combat the spread. Multiple methods are employed for diagnosis and identification of viral infections including viral culture, antigen detection, nucleic acid amplification test (NAAT), serology, rapid diagnostic test (RDT) for POC settings etc., of which RT-PCR is the gold standard with high sensitivity and specificity. However, the feasibility of their use during a pandemic presents certain challenges. In this perspective, we intend to emphasis on the development, logistics and implementation of rapid diagnostic during public health emergencies in context to the lessons learnt from the current COVID-19 pandemic.
