Zusammenfassung

Die Therapie metabolischer oder respiratorischer Azidosen mit Puffersubstanzen ist keine kausale und in nicht in allen Fällen eine hilfreiche Behandlungsmaßnahme. Kapitel. 14 beschreibt die Vorzüge und Nachteile therapeutischer Puffersubstanzen. Für die beiden verfügbaren Puffersubstanzen Natrium-Bikarbonat und Trometamol werden die Ergebnisse von Laboruntersuchungen, Tierexperimenten und klinischen Studien, Nebenwirkungen, Indikationen und Kontraindikationen sowie Dosierungsempfehlungen dargestellt. Weiterhin werden Informationen zu alternativen Puffersubstanzen und Metaboliten gegeben, die entweder nur in klinischen Studien bzw. außerhalb der Zulassung angewendet wurden, wie Carbicarb, Tribunat und Dichlorazetat, oder kurz vor der Zulassung stehen, wie Veverimer. Als Notfallbehandlung werden auch Hämodialyse und Hämofiltration aufgeführt. Zuletzt werden für jede der vier Störungskategorien des Säuren-Basen-Haushalts alle in Frage kommenden Behandlungsmaßnahmen zusammengefasst.
