Zusammenfassung

Die fachgerechte mikrobiologische Infektionsdiagnostik ist die Grundlage für die Diagnose von Infektionskrankheiten und für eine zielgerichtete individuelle antimikrobielle Therapie. Ärzte und Einsender brauchen die Gewissheit, dass die mikrobiologischen Befunde richtig, signifikant und klinisch relevant sind. Um die Qualität der mikrobiologischen Untersuchungen zu erhöhen und die Laboranalysen und Interpretation zu optimieren, muss gewährleistet sein, dass alle Proben für die mikrobiologische Diagnostik ordnungsgemäß abgenommen, gelagert und transportiert wurden und im Labor rasch und zuverlässig analysiert worden sind. Biomarker in Infektion und Sepsis sind eine sehr heterogene Gruppe von Substanzen, wie Proteine, Zytokine, Bakterienprodukte, lösliche Zelloberflächenprodukte und vieles mehr. Wegen der komplexen Pathophysiologie von Infektion und Sepsis sollte der ideale Marker frühzeitig und auch im Verlauf eine erneute Interventionspflichtigkeit durch erweiterte Diagnostik oder in der Chirurgie die Notwendigkeit einer Operation anzeigen. Das Prinzip von Biomarkern soll erläutert, ihre Beziehung zur infektiösen und septischen Krankheit soll dargestellt und unterschiedliche Typen sollen in einer Auswahl vorgestellt werden.
