Zusammenfassung

Die offene Adenomenukleation bietet über einen transvesikalen und transkapsulär-extravesikalen Zugang die Möglichkeit einer sicheren und vollständigen Adenomentfernung der Prostata. Das über Jahrzehnte bei größeren Prostatavolumina (>80 ml) als Standard angesehene Verfahren mit einer vergleichbar kurzen Operationszeit und geringen Re-Interventionsrate kommt aufgrund der heute gut verfügbaren minimal-invasiven Alternativen, insbesondere der Laserverfahren, zunehmend weniger zum Einsatz und bleibt insbesondere Fällen mit Begleitpathologien wie Blasensteinen und therapiebedürftigen Harnblasendivertikeln vorbehalten (Madersbacher et al. 2005; Gratzke 2007; Noguera und Rodriguez 2008).

Lymphozelen, rekto-urethrale Fisteln und Anastomosenstrikturen stellen seltene Komplikationen nach radikaler Prostatektomie dar, sind jedoch für den Patienten häufig mit zusätzlichen operativen Interventionen sowie einem langwierigen zusätzlichen Therapieverlauf assoziiert. In dem beiliegenden Beitrag beschreiben wir die zielführende Diagnostik und Therapie dieser drei operationsassoziierten Komplikationen mit den verschiedenen chirurgisch-rekonstruktiven Behandlungsansätzen.
