Zusammenfassung
Frauen im reproduktiven Alter, die an einer Epilepsie erkrankt sind und regelmäßig Antiepileptika einnehmen, sind mit Fragen der Sicherheit der Kontrazeption und möglicher Risiken durch Anfälle und Antiepileptika während der Schwangerschaft konfrontiert. Hormonelle systemisch wirksame Kontrazeptiva sind insbesondere bei Einnahme von enzyminduzierenden Antiepileptika nicht ausreichend sicher. Hormonelle Intrauterinsysteme oder hormonfreie Barrieremethoden sind mögliche Alternativen. Systemische hormonelle Kontrazeptiva können den Abbau von Antiepileptika, wie Lamotrigin, beschleunigen und das Anfallsrisiko erhöhen. Fast alle Schwangerschaften bei Frauen mit Epilepsie verlaufen komplikationslos, einige Antiepileptika, vor allem Valproat, bergen aber ein hohes teratogenes Risiko. Manche Antiepileptika werden in der Schwangerschaft rascher verstoffwechselt, sodass deren Dosis angepasst werden muss. Stillen stellt in der Regel kein Risiko dar.
