Abstract

Appropriate staging of lung cancer using the TNM classification creates a universal language for medical providers when describing lung cancer. Multiple international societies have adopted the recently updated guidelines by the International Association for the Study of Lung Cancer and provide a framework for clinicians. Staging is essential in helping dictate treatment in a particular patient. The staging process begins with a history and physical, a computerized tomography (CT) scan, and, frequently, a positron emission tomography (PET) scan. Invasive staging is required for patients at higher risk of nodal disease, including those with large or central tumors and those with lymphadenopathy. Tissue sampling for staging and diagnosis can be done using endosonography, mediastinoscopy, and/or surgery; each method has limitations in accessing lymph nodes and peripheral nodules with the goal of obtaining enough tissue for staging, diagnosis, and molecular testing as indicated. In patients who undergo surgical resection, final pathological staging supersedes clinical staging.
