Abstract

PACE is able to provide outstanding end-of-life care to members due to relationships that are developed between the participant and PACE team. PACE is able to provide similar services as the Medicare Hospice Benefit, while maintaining continuity with the primary care team. PACE is also able to provide palliative care services prior to end of life with greater focus on symptom management among this high-risk population of older adults with multiple medical problems and shortened life expectancy.
