Transjugular Intrahepatic Portosystemic Shunt (TIPS)

Abstract

A Transjugular Intrahepatic Portosystemic Shunt (TIPS) involves creating a low-resistance connection between the systemic and portal venous systems within the liver. This is accomplished by forming a tract between an inflow portal vein and an outflow hepatic vein, effectively lowering the pressure within portal system in the setting of acute or chronic complications of portal venous hypertension.

Now a common treatment offered to patients with hepatic cirrhosis, the initial concept of the TIPS procedure grew out of accidental punctures of the intrahepatic portal branches during transjugular cholangiograms. Further developed by Dr. Josef Rösch at UCLA in the late 1960s, the clinical utility of TIPS was circumspect due to difficulties with long-term patency. As stents became commercially available, TIPS became a more viable therapeutic option throughout the 1980s and beyond.
