Aneurysmal Subarachnoid Hemorrhage and Endovascular Treatment: Usefulness of Transcranial Doppler (TCD/TCCS) for Cerebral Hemodynamic Monitoring

Abstract

Subarachnoid hemorrhage is a devastating disease with high morbidity and mortality and multiple neurological and systemic complications. Delayed cerebral ischemia due to vasospasm is the most frequent neurological complication, and related symptoms may develop in up to 30% of cases.

Transcranial Doppler (TCD) is an accessible tool, applicable for monitoring cerebral blood flow velocities (CBFVs) and hemodynamic indexes derived from them. Its characteristics make it very useful for the detection and monitoring of vasospasm, which is crucial for the clinical management of patients with this pathology.

Likewise, its applicability extends to the diagnosis of non-ruptured intracranial aneurysms, and in association with other imaging techniques, it can be useful in the evaluation of recanalization of previously treated aneurysms. For small aneurysms, the use of echo-contrast increases the diagnostic sensitivity of TCD. The incorporation of new technologies could extend its application to other fields such as intraoperative monitoring.
