Postoperative Complications of Cranial Chronic Subdural Hematoma

Abstract

Surgical evacuation of intracranial chronic subdural hematoma (CSDH) has been performed for long years by neurosurgeons worldwide with good results. However, this well-known disease has some potential postoperative complications that can adversely impact morbidity and especially mortality. Overall reported incidences of these complications vary in literature between 5 and 38% of operated patients for CSDH. Complications can be related to surgery or surgical technique, while others are nonsurgical (common medical/systemic) complications. Many options exist for the management of all these postoperative problems, but none of them can be considered standard of care. Great caution must be taken perioperatively and during the surgical procedure to reduce postoperative complications of this common disease. Also, main surgical risks and complications should be clearly explained to the patient or their family before surgery. This chapter provides a comprehensive overview of the main complications that are likely to occur after surgical evacuation for intracranial CSDH, including recurrence, subdural empyema, new acute intracranial bleeding, seizure, tension pneumocephalus, and nonsurgical (medical) complications. I hope these pieces of information will be helpful in the day-to-day practice of cranial surgery for CSDH.
