Abstract
Key features of orbital multiple myeloma (MM):
-
Median age at diagnosis: 70 years
-
Equal gender distribution
-
Majority already diagnosed with MM
-
Unilateral bony tumors (involving the sphenoid or frontal bone with orbital extension into the superior and superotemporal orbit)
-
Other anatomical patterns include:
-
1.
Discrete orbital tumors (mostly intraconal and adhering to an extraocular muscle)
-
2.
Trans-spatial/multi-compartment lesions (e.g., sinuses, eyelids and facial soft tissues)
-
1.
Imaging features on CT and MRI are variable:
-
The greatest value in imaging is staging the local extent
-
Enhancing soft tissue mass on CT with possible bony erosion, destruction or expansion
-
Nonspecific MRI signal characteristics:
-
T1-weighted images: isointense signal intensity to muscle
-
T2-weighted images: intermediate to high signal intensity, often homogenous
-
Post-contrast enhancement
-
Restricted diffusion due to hypercellularity
