Periocular Fillers–Related Complications: Imaging Features

1 Signs and Symptoms

Each complication has its own distinctive features; these commonly include local swelling and inflammation. In cases of vascular compromise, pain, paresthesia, erythema, blisters, skin necrosis, and vision loss can occur.

2 Differential Diagnosis

Filler-related complications may mimic:

  1. 1.

    True pathology (primary/recurrent tumor). Both may demonstrate enhancement/increased uptake on FDG PET-CT/MRI. Restricted diffusion on DWI and atypical location for filler injection should prompt a potential for malignancy and warrant biopsy

  2. 2.

    Dermatological entities, including sarcoidosis, cutaneous lymphoma, and dermatomyositis

3 Diagnosis

  • MRI is the preferred modality for diagnosis, anatomic delineation, and differential diagnosis of filler-related complications especially in unclear cases and in long-term filler-related complications.

  • CT is superior for identification of calcifications, which are pathognomonic of certain filler (CaHA) and certain filler-related complications.
