Drug Resistance in Hepatocellular Carcinoma

Abstract

At present, the burden of cancer morbidity and mortality is increasing all over the world. Among all kinds of cancer, primary liver cancer is discovered to be the seventh most common cancer and has an extremely high death rate, affecting countless patients. Globally, the main histology of primary liver cancer is hepatocellular carcinoma (HCC), characterized by pathological changes in parenchymal cells of the liver, accounting for over 70% of liver cancer. Currently, HCC treatment mainly includes surgical intervention, local ablative therapy, and local therapy. Sorafenib, a multityrosine kinase inhibitor, is approved for use in patients with unresectable HCC. Tyrosine kinase inhibitors (TKIs) (lenvatinib, regorafenib, and cabozantinib) and a VEGF receptor inhibitor (ramucirumab) have also been used for HCC treatment. Although drugs for HCC treatment have been under further development, the acquired drug resistance increasingly affects the management of HCC patients. Researchers have focused on drug resistance for many years and identified complex regulatory networks involved in this issue. In this chapter, the significant research progress regarding mechanisms underlying HCC drug resistance is summarized, which may benefit the treatment of HCC patients and give future direction for new drug development for HCC.
