What Have we Learned from 10 Years Peripheral Intravascular Ultrasound?

Summary

Although angiography has been considered as the ‘gold standard’ in the assessment of the human vasculature, the technique only displays the contrast-filled lumen and gives no information on the arterial wall. The necessity for more detailed information on the arterial anatomy accelerated development of high frequency intravascular ultrasound (IVUS) systems in the 1980s. It soon became evident that, compared with other imaging techniques, the capability to provide information on the arterial wall is a unique advantage of IVUS. This chapter will answer questions related to 1) the accuracy of IVUS as assessed in in vitro studies and 2) the value of the clinical use of IVUS in peripheral arteries, as experienced in the last 10 years at the authors’ center. Results from studies in the femoropopliteal artery, the renal artery and the abdominal aorta will be presented. In addition, the role of three-dimensional (3-D) reconstruction of IVUS will be discussed.
