Zusammenfassung

Während die kontinente Ureterosigmoidostomie oder Harnleiterdickdarmimplantation die älteste Harnableitung überhaupt ist und bereits im 19. Jahrhundert erste Anwendung fand, sind kontinente hetero- oder orthotope Harnableitungen noch relativ jung. Diese werden seit Ende der 70-er Jahre eingesetzt, beginnend mit dem kontinenten, katheterisierbaren, heterotopen Pouch aus welchem sich Mitte der 80-er Jahre der orthotope Blasenersatz weiterentwickelt hat. Die zwischenzeitlich vorliegenden Langzeitergebnisse bestätigen gute funktionelle und onkologische Resultate, wobei jedoch beim heterotopen, katheterisierbaren Pouch eine nicht ganz unerhebliche Reoperationsrate zu verzeichnen ist. Nicht nur deshalb sondern auch aus verschiedenen, meist praktischen, physiologischen und psychologischen Gründen wird der orthotope Blasenersatz weltweit immer häufiger angewendet und gilt v. a. bei jüngeren Patienten als präferierte Harnableitung. In den letzten Jahren hat die Zahl der kontinenten hetero- resp. orthotopen Harnableitungen aus verschiedenen Gründen (zunehmendes Alter der Patienten; roboter-assistierte, rein intrakorporelle Harnableitung) wieder etwas abgenommen.
