Zusammenfassung
In jedem Bereich der Medizin gilt es als unerläßlich, daß vor einer Behandlung Diagnosen gestellt und daß nach einer Therapie Erfolge kontrolliert werden; bleibt das erwartete Ergebnis aus, muß die Behandlung geändert werden. So wie der Chirurg weder eine Operationsindikation noch die Überwachung der Wundheilung (ausschließlich) dem Pflegepersonal überläßt, darf der Anästhesist sich nicht damit begnügen, nach Abschluß einer Narkose vage Verordnungen zur postoperativen Schmerzbehandlung aufzuschreiben, ohne deren Wirkung beim Patienten zu überprüfen und die analgetischen Maßnahmen ggf. zu modifizieren. Aus einer Arbeit von Keats geht hervor, daß postoperative Beschwerden nicht einfach mit dem Wundschmerz gleichgesetzt werden können [91] (Tabelle 1). Es muß einleuchten, daß bei ätiologisch ganz unterschiedlich bedingten Symptomen auch spezifische Therapien erforderlich werden; hierzu ist allerdings zunächst eine korrekte Diagnose erforderlich.
