Abstract

A refractive surgeon should be able to diagnose and manage uncommon cases of late-onset diffuse lamellar keratitis (DLK) in patients with epithelial defects after microtrauma. DLK is characterized by an inflammatory cellular response at the interface after lamellar corneal surgery, particularly LASIK [1]. It usually occurs during the first week following surgery. Various factors act as etiologic agents including blade debris, bacterial endotoxins on instruments, meibomian gland secretions, and epithelial defects during surgery [2–6]. Late-onset DLK is also associated with anterior segment inflammatory conditions such as allergic and adenoviral conjunctivitis or iritis [7].
