Endoscopic Calcaneoplasty

Abstract

The first case of a patient with posterior heel pain, caused by hypertrophy of the posterosuperior part of the calcaneus in combination with wearing rigid low-back shoes, was desc-ribed by Haglund in 1928.1 Nowadays in Haglund’s disease, we describe a clinical situation of tenderness and pain of the posterolateral aspect of the calcaneus. On physical examination, a bony prominence can be palpated at this location. This entity is described by a variety of different names such as “pump-bump,”2 cucumber heel,3 winter heel,4 etc. In Haglund’s syndrome, the retrocalcaneal bursa is inflamed and therefore swelled, sometimes in combination with insertional tendinopathy of the Achilles tendon. The syndrome is caused by repetitive impingement between the anterior aspect of the Achilles tendon and the enlarged posterosuperior aspect of the calcaneus. The patient typically describes the onset of pain when starting walking after a period of rest. Operative treatment consists of removal of the inflamed retrocalcaneal bursa and resection of the bony calcaneal prominence.
