Management of Chronic Constrictive Pericarditis

Surgical Treatment of Chronic Constrictive Pericarditis

Abstract

Since establishment of diagnosis of constrictive pericarditis continues to elude the clinicians, we searched the extant literature and collated various clinical and investigative parameters used for establishment of the diagnosis. Analysis of the published literature substantiates that atleast 2 studies are needed namely, echocardiography, cardiac magnetic resonance, and/or cardiac catheterisation to conclusively establish the diagnosis of constrictive pericarditis. The difficulty in making a histological or bacteriological diagnosis of tubercular constrictive pericarditis, usefulness of anti-tubercular chemotherapy with/without adjunctive corticosteroids in tubercular pericardial effusion, the timing of pericardiectomy for post-transplant constrictive pericarditis, effusive-constrictive pericarditis and utility of cardiopulmonary bypass to achieve total pericardiectomy have been highlighted.
