Arthroscopic Suture to Loop Fixation of PCL Tibial Avulsion Fracture

Abstract

PCL avulsion fracture is a special kind of PCL injury and needs to be surgically addressed when the displacement or joint instability is severe [1]. Both open and arthroscopic procedures have been reported, and similar clinical results have been obtained [2, 3]. Regarding open procedures, the fixation is direct and simple, and there is no need of the learning curve of arthroscopic surgeries [4–7], but it is invasive and the antegrade fixation devices placed through open procedures can only be removed through an additional invasive open procedure when needed. The advantage of arthroscopic procedures lies in that the fracture can be directly exposed intraarticular and combined intraarticular lesions can be treated simultaneously, and its mini-invasive nature. Though the learning curve of arthroscopic procedures is relatively long, it has long been tried [8–10]. Regarding the specific techniques, pullout fixations through tibial tunnels are most reported recently [11, 12]. Thus, we would like to introduce an arthroscopic fracture reduction and fixation technique, in which fracture reduction and fixation is performed with pulling out of the ligating sutures and fixation at an adjustable suture loop. The indications of this technique are acute PCL tibial avulsion fracture with fracture displacement or posterior instability reaching over 8 mm.
