Nephron-Sparing Surgery

Wilms’ Tumor

Abstract

Nephron-sparing surgery has slowly and steadily ascended to its present position when it is no more considered as a mere tentative option of surgical management of Wilms’ tumor, but a validated surgical principle that could give the radical nephrectomy run for its money in the coming future. It is no more indicated only for bilateral Wilms’ tumors and syndromic Wilms’ tumors, but is also performed in non-syndromic unilateral Wilms’ tumor. It has been now known to have similar if not better oncological outcomes, but also leads to better long-term renal functions and lesser incidence of hypertension. The procedure is advised in early stages of the disease, though metastatic disease is not an absolute contraindication. This chapter describes among others its historical background and different operative techniques.
