This important chapter details a collection of challenging cases which required innovative/uncommon solutions. It is hoped that this chapter underscores the vital importance of careful individualization in each case, starting with a sound excision plan.

The following cases are presented:
  1. 1.

    Massive, recurrent squamous carcinoma of the scalp with calvarial invasion

  2. 2.

    A case of extensive dermatofibrosarcoma protuberans of the forehead, extending into the eyelids and glabella

  3. 3.

    Locally advanced carcinoma of the lachrymal sac with bony invasion of the orbital walls and proptosis

  4. 4.

    Extensive soft tissue tumor of the retromaxillary space and infratemporal fossa in a 16-year-old boy, reported initially as synovial sarcoma

  5. 5.

    Large squamous carcinoma over the nasal dorsum and cheek in a 75-year-old lady

  6. 6.

    Recurrent high-grade mucoepidermoid carcinoma of the parotid with invasion into the zygomatic arch and infratemporal fossa

  7. 7.

    Extensive mucoepidermoid carcinoma of the parotid tail with invasion into neck muscles

  8. 8.

    Large adenoid cystic carcinoma involving the entire base of tongue as well as the lingual septum in a young lady

  9. 9.

    Locally advanced squamous carcinoma invading the base tongue, vallecula, and supraglottic larynx in a young individual

  10. 10.

    Large, nodular T3 squamous carcinoma of the supraglottis and pyriform sinus with airway inlet compromise

  11. 11.

    Locally advanced carcinoma cheek (inoperable at primary presentation) with skin and mandible invasion
