Abstract

The basic features of laparoscopic colorectal cancer surgery are described herein. The completeness of the surgical technique must begin with the definition of the terms used in the procedure. The surgical anatomy derived from embryology must then be understood. The understanding of the embryology must be transposed to the clinical setting. In order to understand the overall picture of the colon, the relationship between the stomach and the colon cannot be ignored. Many existing texts have incorrectly described this area. With reference to intestinal rotation and fusion fascia in this area, it is possible to understand the fascial composition. Definition of the vascular anatomy in the colon has not previously been attempted. While the gross appearance of the vasculature has been considered, lymph node dissection along the vessels alone has been emphasized. It is assumed that detailed understanding of vascular anatomy is necessary, followed by evaluation of lymph flow. Thus, it is necessary to review the definitions of lymph node dissection.
