Abstract
Twenty years ago case managers were relatively rare creatures; but now many community-based healthcare agencies have a member of their team who has some level of case management or care coordination as their primary role. Everyone wants to live a good life and has the right to try and live their life the way they want. Sometimes chronic healthcare issues, disability, psychosocial challenges and complex and changing services can interfere with such goals.
