Clinical Ethics Handbook for Nurses

Abstract

Persons with mental health concerns are encountered in all areas of nursing practice. While this chapter specifically addresses ethical issues in psychiatric and mental health nursing, the information is pertinent for nurses across settings. A major issue in psychiatric and mental health settings is the problem that cognitive difficulties can interfere with making reasoned choices and processing information. We discuss ways to mitigate the harm such difficulties can cause to a person’s sense of dignity and even physical safety. We also discuss the limits that have to be placed on a person’s freedom when others are at risk of harm from their actions. The roles of stigma and bias and their compounding influences on mental health are explored along with mindfulness strategies to facilitate recognition and control of implicit and explicit biases. Additionally, the evolving role of telehealth and personal data storage are examined related to how sensitive information is protected. An important responsibility of nurses related to individuals with unmet mental health needs is addressing problems with access to mental health care. Various approaches to influencing problematic policies are suggested.
