Abstract

Malignancies of the foregut, including, gastric and esophageal cancers, carry a heavy healthcare burden. Gastric cancer is one of the leading causes of cancer-related mortality worldwide. While surgical resection remains the cornerstone of curative intent treatment, most patients present with locally-advanced or metastatic disease, requiring systemic therapy. The high toxicity and variable response rates of standard cytotoxic systemic chemotherapy leave much room for improvement in the management of patients with these devastating diseases. The foundation for tackling these cancers lies in our understanding of molecular events underlying their etiology and pathogenesis. The appreciation of the role of translation and its regulation in gastric and esophageal cancers is growing and, while still in its infancy, research in this area highlights that various hubs of translation are involved. Namely, dysregulation in expression and activity of proto-oncogene eIF4E and its inhibitory mTOR responsive molecules, 4E-BPs, are found in these types of cancer. Other components of the translation machinery, such as tumor suppressor PDCD4, also appear to play an important role in gastric and esophageal cancers. The role of these factors, their regulation, and therapeutic targeting in upper gastrointestinal tract malignancies will be further detailed in this chapter.
