In situ immunophenotyping of follicular keratinocytes in alopecia areata

Abstract

Histological and immunological findings indicate that alopecia areata (AA) is an immunologically mediated disease. Previous investigations could demonstrate the expression of the MHC class II antigen HLA-DR on follicular keratinocytes in the progressive stage of AA. The expression of MHC class II antigens by epithelial cells represents a reaction of unknown function, and it is not clear whether the expression of HLA-DR by follicular keratinocytes is a primary or a secondary event in the pathogenesis of AA. Nevertheless this was considered as an indication for the involvement of the follicle in immunological reactions, and as a confirmation of the immunological nature of this disease. Meanwhile we know that keratinocytes in diseased or healthy skin can react with different monoclonal antibodies against MHC class II antigens. For this reason we investigated the reactivity of follicular keratinocytes with a series of monoclonal antibodies against MHC class II antigens HLA-DR, -DQ, -DP in situ. Scalp biopsies from 40 patients with AA were compared with biopsies from normal individuals using the avidin-biotin-immunoperoxidase method.
