Abstract
Correlation between the clinical features of alopecia areata including therapeutic response to squaric acid dibutylester and histopathological changes was studied using the biopsied scalp skins from 23 patients. Thirteen cases which showed no therapeutic response exhibited a reduced size of anagen follicles (microfollicles) showing dystrophic changes in the suprabulbar portion of the follicles accompanied by dense infiltration of lymphocytes in the perifollicular dermis. Moreover, these patients had a long duration, over 5 years, of symptoms. Eight cases which showed either good or moderate response had medium to normal sized anagen follicles and most of them showed mild pathological changes in the follicular milieu Longstanding refractory lesions cause regressive changes of the hair follicles concomitant with latent inflammatory changes in the dermis.
