Progestins in the peri-menopause

  • Chapter
The Controversial Climacteric

Abstract

Since the first retrospective studies indicated in the mid-1970s that the risk of endometrial cancer from post-menopausal oestrogen use was increased 4.6 to 8-fold, a 10–14-day course of progestogens has been advocated for routine usage with oestrogen replacement therapy (ERT) (Gambrell, 1978). Progestogens have, however, been shown to have a potentially adverse effect on both carbohydrate and lipid metabolism. The possibility therefore exists that the prevention of one disease (endometrial cancer) might provoke another (atherosclerosis), thereby negating the benefit of the currently advocated hormone replacement regime. This workshop set out to redefine the efficacy and need for combined oestrogen/progestogen therapy, and to evaluate the potential side-effects.
