Evaluation of non-glucose carbohydrates in parenteral nutrition

Abstract

If those who required parenteral nutrition were healthy there would be no need to discuss the choice of carbohydrate because glucose is the main energy substrate for the brain. It prevents excessive losses of sodium and water and stimulates the secretion of insulin, thus enhancing anabolism. However, in patients who are very ill or who have sustained a severe injury or operation, there is an increased secretion of adrenaline and noradrenaline which inhibits insulin release. Glucose metabolism could be controlled in most of these conditions by the careful administration of insulin. This is not always accomplished however even by experts. Hence the search for carbohydrates and carbohydrate-like substances which are metabolized independent of insulin.
