Abstract

Although aorto-coronary artery bypass grafting is a relatively recent innovation, surgery for the ischemic myocardium has been undertaken since the early 1900s. These early procedures included cervical sympathectomy [1], indirect revascularization by promotion of epicardial collaterals (Beck [2] and Vine berg [3] procedures), and coronary sinus arterialization (Beck II procedure [4].) It was not until the 1950s that direct coronary revascularization was first performed, initially by coronary endarterectomy [5] and later by direct aorto-coronary bypass procedures [6,7]. In 1966, Favaloro [8] from the Cleveland Clinic and Johnson [9] from Milwaukee, popularized routine coronary artery bypass grafting. Since then, approximately 300 000 aortocoronary artery bypass procedures [10], as well as a similar number of coronary angioplasties [11], are performed annually in the United States. The efficacy of these procedures, both in the relief of symptoms and in the prolongation of life, have been the subject of much debate. In this chapter we will address long-term results of coronary revascularization by coronary artery bypass surgery, with emphasis upon survival, symptom relief, and overall quality of life.
