Abstract
The single most dramatic advance in the treatment of brain metastasis has been the introduction of adrenal glucocorticoids. These drugs, first used to treat brain metastasis by Kofman and his colleagues (1957), have revolutionized the initial treatment of metastatic brain tumor and have at times restored moribund patients to full neurologic function, often within hours. Although glucocorticoids are now used to treat several brain disorders, in no other illness is the response so consistent and so dramatic as in treating brain metastases (Fishman 1975; Reulen 1976). This chapter reviews the effects of glucocorticoids, both salutary and untoward, on patients with brain metastases and discusses the mechanism(s) by which these agents exert their beneficial effect.
