Abstract

I will summarize the new concepts concerning the metabolism of surfactant, the available information on the physiologic and biochemical effects of surfactant therapy, and some unique features of the developing lung that influence the strategies used to treat lung disease in the infant. While these observations are directed toward the responses of the immature lung to ventilation, the same events no doubt occur in the mature lung in disease, although their relative importance has not been defined. A developmental perspective toward lung disease and its therapy is a rewarding approach that will yield insight into the processes of lung disease in the older child and adult.
