Abstract

Gas exchange depends on the state of the lungs—the exchange organs—and two pumps, the heart and the respiratory pump. The heart pumps blood through the lungs to exchange with the air brought into the lungs by the second pump, the ventilatory pump. Failure of any one of these three elements, or insufficiency and deficiency in a combination of these elements can result in respiratory failure. Many errors in patient management result from concentration on one element of the system to the exclusion of the others. Effective therapy depends on analysis of the element or elements which are not functioning adequately and providing therapy appropriate to the specific failures.
