Hepatitis B versus Dioxin (TCDD) in the Pathogenesis of Excess Primary Liver Cell Carcinoma in Viet Nam

Zusammenfassung

In Viet Nam stieg in den letzten Jahren die Häufigkeit des primären Leberzellkarzinoms (HCC) sprunghaft an, besonders in den Gebieten, die während den II.Indochinakrieges intensiv mit dem mit Dioxin (TCDD) verunreinigten Herbizid “Agent Orange” besprüht worden waren. Da nur bei älteren Karzinomträgern eine Korrelation mit persistierender viraler Hepatitis B nachzuweisen war und da in Viet Nam die sonst in tropischen Ländern als weitere Hauptverursacher des HCC angesehenen Faktoren wie Mykotoxine oder Schistosomiasis und Opisthorchosis fehlen, wird die Rolle des TCDD in der Genese des HCC vor allem bei jüngeren Patienten diskutiert. TCDD könnte entweder direkt auf Grund seiner Hepatotoxizität oder indirekt als “promoting Agent” wirken, das im Rahmen der Zweiphasentheorie nur die morphologische Manifestation einer vorangehenden malignen Transformation induziert.
