Hepatic arterial infusion chemotherapy: rationale, results, credits and debits

Abstract

The chemotherapy of primary and secondary neoplasms of the liver has evoked over the past decades passing through phases of single agent systemic treatment, regional infusion chemotherapy, combination chemotherapy and finally continuous regional drug delivering. This latter development has been made possible through the development of a very sophisticated biomedical device: the totally implantable pump. Since it’s introduction for continuous hepatic arterial chemotherapy in 1980 it is estimated that over 5000 implantations have been performed of the Infusaid® pump, suggesting a therapeutic fad in the treatment of liver metastases. Although the Infusaid pump represents a substantial technological advance, it is important to distinguish the device itself, it’s reliability and the role it plays in regional chemotherapy, from the efficacy of the regional approach to metastatic colorectal cancer in the liver. This chapter will mainly focus on this latter complex problem in which the pump is merely a device for the delivery of agents which, like the method itself, will be subject to a critical review of credits and debits.
