Surgery for ventricular arrhythmias is still the most challenging surgical approach in the treatment of cardiac arrhythmias. In the operating room, the pathophysiology of the lesions is still obscure, and the surgical rationale has to compromise with the imperative preservation of the left ventricular function. In this review we will try to provide some perspective on what we have learned in the clinical setting about the surgical treatment of ventricular arrhythmia based on empirical or on experimental models.
