The use of exercise testing in Comprehensive Coronary Care (C.C.C.)

For some mysterious reasons, which remain unclear, the rehabilitation of cardiac patients has been considered for decades as equal to physiotherapy and as a kind of psychological intervention. Clinical cardiology has shown an unfounded dogmatic aversion against rehabilitation and it has often been stated that there is a lack of scientific proof to show that cardiac rehabilitation influences longevity and prevents reinfarction. Despite the fact that a number of randomized trials have been undertaken in the past decade, only the so-called historically controlled, but not randomized studies, have demonstrated that there is a decreased mortality in the intervention groups. Randomized studies also had a beneficial effect, at least in the short term (up to 4 years), but it has proven almost impossible to continue these studies for the last few years due to various problems concerning compliance, drop-outs, drop-ins and contaminations [1]. It was also impossible to avoid a number of other biases which would influence the studies outcome and the approach fact that cardiac rehabilitation is identical to comprehensive multifactorial therapeutic approach has been neglected.
