Long-term double filtration LDL-apheresis in familial hypercholesterolemia

Abstract

Direct removal of LDL from plasma by plasmapheresis is an available method of treatment in FH homozygous patients and in the FH heterozygotes resistant to drug therapy.

Double filtration (DF) permits the removal of plasma components with molecular weight greater than about 300 kd.

We report the results of DF long-term therapy in 3 FH patients:one man (G.D.),aged 40 yr, and two women (C.S. and L.M.) aged 23 yr.All three had positive pedigree for FH,xanthomatosis since the age of 10,mean plasma total cholesterol levels ranging from 600 to 750 mg/dl. DF was repeated every two weeks,and the total amount of plasma treated by each procedure was 3 litres.

DF causes a remarkable decrease in plasma total cholesterol (-53%),LDL- holesterol (-55%),triglycerides (-54%) and HDL-cholesterol (-40%). During the DF therapy a progressive reduction of Achilles tendon thickness and of tuberous xanthomata volume was recorded.

No significant side effects have been observed.
