Samenvatting

De omslag van ‘jong-ziek’ naar ‘oud-ziek‘ heeft een omwenteling in het dokteren teweeggebracht. De enkelvoudige problematiek is naar de achtergrond verdwenen en complexe problematiek is routine geworden. Met de vlucht naar voren van de specialistische disciplines is grote vooruitgang geboekt, maar de toenemende meerwaarde valt terug met toenemende nevenwerkingen en uit de hand lopende kosten als gevolg. De nadruk op het volgen van richtlijnen heeft perverse effecten, omdat voor de meeste oudere patiënten van vandaag geen op evidentie geschoeide richtlijnen voorhanden zijn. De oplossing ligt in kennisoverdracht, het scheppen van vertrouwen, luisteren en terugkoppelen, waarmee de dokter de regie over gezondheid en ziekte aan de patiënt teruggeeft en daarmee een langdurig succes probeert te verzekeren.
