Samenvatting

In dit hoofdstuk wordt het begrip LUTS (lower urinary tract symptoms) geïntroduceerd, ingedeeld in mictieklachten, postmictieklachten en opslagklachten. Bij de ouder wordende man wordt vaker gevraagd: ‘hoe gaat het met het plassen?’ dan ‘hoe gaat het met ophouden?’. Onbedoeld vragen we vaker naar de mictiefase dan naar de opslagfase. Echter beide zijn van belang. De oorzaak van plasklachten bij de ouder wordende man is multifactorieel en moet ook als zodanig worden onderzocht en behandeld. Uitgangspunt is de mictiecyclus bij mensen zonder klachten. De blaas is het grootste deel van de dag in een opslagfase en slechts kortstondige momenten in de mictiefase. Bijpassende LUTS correleren slecht met onderliggende pathofysiologie, maar geven wel richting aan een symptoomgerichte medicamenteuze therapie. Zolang ingezette therapie goed wordt geëvalueerd en alarmsymptomen niet gemist, kan een stappenplan in de eerste lijn als leidraad dienen voor een veilige behandeling van mannen met LUTS zonder over- of onderhandeling.
