Anesthesiologische aspecten van diepe hersenstimulatie

Samenvatting

Diepe hersenstimulatie wordt steeds meer toegepast voor de behandeling van de ziekte van Parkinson en andere bewegingsstoornissen. De anesthesioloog speelt in dit zorgtraject een belangrijke rol. De uitdagingen voor de anesthesioloog met betrekking tot de zorg van de patiënt hangen samen met de specifieke neurologische stoornis, de effecten van de anesthetica op de micro-elektroderecording, en het zorg dragen voor een wakkere en heldere patiënt ten tijde van de peroperatieve klinische testen. Daarnaast moet de anesthesioloog kennis hebben van de mogelijke complicaties die kunnen optreden voor deze specifieke groep patiënten. De te kiezen anesthesietechniek hangt af van de eigen ervaringen en gewoonten en kan per instelling verschillen. Een lokale of regionale anesthesie in combinatie met zogenoemde ‘conscious sedation’, ‘asleep-awake-asleep’ procedure of algehele anesthesie zijn verschillende technieken die kunnen worden gebruikt. Tot op heden is er nog geen gouden standaard met betrekking tot het beste anesthesiologisch beleid. Ongeacht welke techniek wordt gebruikt, blijft de invloed van anesthetica op peroperatieve neuromonitoring een punt van discussie.
