Articaïne en neurotoxiciteit

Samenvatting

Articaïne wijkt wat samenstelling betreft af van andere amide-anesthetica. Bij articaïne bestaat het lipofiele deel uit een thiofeenring, terwijl dit bij de andere anesthetica een benzeenring is.

Gebruik van lokale anesthetica in de tandheelkundige praktijk is bijzonder veilig. Elk lokaal anestheticum is echter potentieel neurotoxisch, waardoor in zeldzame gevallen na het verstrijken van de verwachte werkingsduur nog sprake kan zijn van een verstoorde gevoelssensatie (paresthesie). In diverse landen zijn retrospectieve studies uitgevoerd naar de incidentie van persisterende paresthesie na het gebruik van lokale anesthetica. In de meeste onderzoeken werd een oververtegenwoordiging van paresthesieën gevonden bij gebruik van articaïne ten opzichte van het aantal dat op grond van het marktaandeel van dit anestheticum mocht worden verwacht. In dierexperimenten en celkweekexperimenten heeft articaïne echter geen hogere toxiciteit dan andere anesthetica. Nader onderzoek naar de mogelijke oorzaken van paresthesie bij gebruik van lokale anesthetica lijkt dan ook aanbevolen.
