Samenvatting
Stress-echocardiografie is een algemeen geaccepteerd onderzoek om coronairlijden vast te stellen met een hoge diagnostische en prognostische opbrengst. Induceerbare wandbewegingsstoornissen (ischemie) zijn voorspellend voor coronair gerelateerde complicaties, de afwezigheid ervan is geassocieerd met een laag risico.
Stress-echocardiografie is geïndiceerd wanneer een inspanningselektrocardiogram niet mogelijk of niet interpreteerbaar is, bij twijfelachtige bevindingen, bij hypertensie en linkerventrikelhypertrofie en bij vrouwen.
Vitaliteitsdetectie is een andere toepassing van stress-echocardiografie en voorspelt een goede uitkomst na revascularisatie bij ischemisch hartlijden.
De stress-echocardiografie kan helpen bij de besluitvorming rond patiënten met een hartklepziekte, zoals mitralisinsufficiëntie, transvalvulaire gradiënten bij aortaklepstenose en mitralisklepstenose, en bij het vaststellen van de systolische pulmonaaldruk.
Stress-echocardiografie heeft een vergelijkbare accuratesse als nucleaire perfusiescintigrafie, maar met aanzienlijk lagere kosten en zonder nadelige milieueffecten of radiatie.
Dobutamine wordt in de praktijk vaker toegepast dan inspanningsechocardiografie, maar heeft een iets hoger risico en meer bijwerkingen.
